By submitting a video to The Catholic Health Association of the United States (CHA) for inclusion in the Medicaid Makes it Possible website, you agree to the following:
- The video shall not contain material that violates or infringes another's rights, including but not limited to material that violates privacy, publicity or intellectual property rights, or that constitutes copyright infringement. Without limiting the foregoing, the video must not include third party trademarks, logos, insignia, location signage, photographs, artwork, or sculptures, without the owner’s express prior written permission. Please do not use music that is not in the public domain (i.e., available to publicize and broadcast on a license-free, no compensation basis.) We're not saying this because we don't love music; we do we really do! It's because we don't want to violate the copyright of musicians and artists.
- You have obtained written permission from all persons appearing in the video for you to display their image, likeness or voice in the video and for such image, likeness and voice to be used by CHA in any way that CHA deems fit, including public display of the video. To the extent that anyone under the age of 18 appears in the video, you represent and warrant that the minor's parents consented to the minor's appearance in the video and the public display of the video by CHA.
- You represent and warrant that your video does not and will not defame or otherwise violate the rights of any third party and does not and will not violate any federal, state, or local laws or ordinances.
- You grant CHA a non-exclusive, worldwide, perpetual, assignable, royalty-free license to publish, reproduce, distribute, display, perform, create derivative works, transmit or disseminate the video in all media or platforms whether now known or hereafter invented.
